Born to Sing Vocals, Policies & Procedures:

Thank you for choosing myself, Britni Favero with Born to Sing Vocals, as your voice teacher. I consider it a great privilege to be entrusted with sharing the gift and love of singing with you, and I look forward to helping you in your own vocal journey. Please thoroughly read & initial each section, sign/date at the end, and return this form to me. Thank you!

By signing this agreement, you agree to respect and abide by the policies.

______General Information: Weekly private lessons are offered in 30 or 60 minute sessions, depending on your scheduled preference. Students are expected to arrive for lessons in a timely manner, with required/desired materials. The length of the lesson remains the same, regardless of tardy arrival by Student. Parents and siblings may wait quietly in the waiting area or their vehicle. To discuss student’s progress, parents are welcome to talk with the teacher during the first five minutes of the student’s lesson, or contact the teacher via phone, email, or text message. 

______Cancellations: Please be respectful of the teacher’s time and schedule and inform the teacher at the earliest convenience of any lessons that will be missed. If the teacher is not informed 24-hours prior to a lesson that the student will be absent, you will be charged for that lesson; students are responsible to pay for lessons canceled without a 24-hour notice. No refund will be given for no-shows or tardiness.

______Makeup Lessons: Due to the teacher’s limited availability, makeup lessons cannot be guaranteed. However, if an absence arises due to unforeseen situations such as an accident, sudden illness, or family emergency that can be handled on a case-by-case basis. If the student cannot attend in person, a lesson via Skype/FaceTime during your regularly scheduled time is an option for that week. 

______Teacher-missed Lessons: If I have to miss a lesson, I will reschedule it at the student’s convenience. If, for some reason, we cannot make up a lesson, payment from that lesson will be credited towards the next month’s bill. 

______Payment: Lessons will be billed on a monthly basis at the beginning of each month. Monthly payments are due by the 10 ^th day of the month. Cash, Check, or Venmo are accepted. If writing a check, please make checks payable to: Britni Favero.

______Practice: Regularity of practice is essential to good progress. Students are encouraged to practice daily as the best results and consistency will transpire when done so. 

______Discontinued Lessons: If a student intends to discontinue lessons, please provide a 2-week notice. However, the Teacher respects the Student’s right to end lessons at any time. Note: At the discretion of the teacher, Students may be asked to discontinue lessons at any time if they have unpaid tuition, an excess amount of absences, or if they show lack of interest, etc. 

______ Waiver of Liability; Indemnification: Parent and Student hereby waives, releases, discharges, and covenants not to sue BORN TO SING VOCALS/BRITNI FAVERO or SEEDLING THEATRE COMPANY (“Studio”), its owners, employees, agents, or representatives (“Releases”) from any and all liability, claims, demands, losses, or damages arising out of, or related to, Student’s participation in studio on-site or off-site lessons, events, or activities (“Studio Programs”), whether based in verbal, contract, or tort. Parent and Student agree to indemnify, defend, and hold harmless the releases from any and all claims, demands, losses, attorney’s fees, costs, liabilities, or damages against Studio arising out of Student’s participation in Studio Programs. 

______ Media Consent: Parent and/or Student hereby agrees and consents to and authorizes the Studio, staff, agents, or other guests to capture and use video and/or audio recordings and photographic images of Student and Student’s musical performances and quoted statements in print, electronic, or online media, for educational and Studio promotional purposes only, without any compensation to Parent or Student. Parent and/or Student agrees to waive and release any and all claims Parent may have against Studio relating to the use of these recordings, images, or statements. This Consent is binding upon all heirs and future legal representatives. Studio will not personally identify a minor child by full name in connection with a video recording or photographic image of the child, unless Studio receives separate written signed consent from the Parent. (If this is left blank, or marked “no”, it will be assumed the Parent or Student does not give Media Consent).

______ Severability: If any term or provision of this Studio Policy and Agreement is illegal or invalid for any reason, such provision is severable from the rest and shall not affect the legality or validity of the remainder of this Studio Policy and Agreement.
